WTC Admission Criteria & Referral Procedures

Admission Criteria

Individuals referred to WTC must meet the following criteria, per RSM 2, Section 1204:

  1. The services and resources needed to address the functional limitations presented by the applicant are available at WTC or are available through community providers in the area and can be coordinated by the Center case manager.

  2. The individual is registered with DORS and meets Order of Selection criteria (see RSM 2, Section 500); is registered with a representative of a public or private organization or agency when an agreement has been established between WTC and the organization or program regarding direct referral of an individual.

  3. There is evidence that the individual has the capacity for self-directed activity and participation in a program of services conducted in a group setting.

  4. The current health status is sufficiently stable to permit participation in a program of VR and/or independent living services.

  5. There is evidence of recovery for individuals with addictions disabilities, to include past/current treatment for addictions.

  6. The admission is voluntary on the part of the applicant.

Referral Procedures

Complete a Service Authorization in the AWARE™ Participant Module to make a referral to WTC. All required referral documentation must be attached to AWARE™. Once the Service Authorization and referral documentation are reviewed by the Admissions Office, a Case Manager will be assigned. Any questions regarding the status of a Service Authorization can be answered by reviewing the Authorization in the AWARE™ Service Module or by communicating with the Case Manager. See RSM 2, Section 1200 for additional information.

Procedures to refer individual:

  1. Follow AWARE™ procedures to complete a Service Authorization.

  2. Include relevant medical information (must include at least one of the following):

    1. General medical examination (GME)

    2. Psychological evaluation and/or current treatment information

    3. For individuals with psychiatric disabilities who will be residing at WTC for more than two weeks, current (within 12 months) documentation of psychiatric status or Case Note describing counselor observations based on DORS Intake Form (RS-1c), initial interview or other interactions.

    4. Other (i.e., medical specialist report, hospital or clinic reports, report by family physician).
