Program Description

A limited range of home modifications may be provided if required by the DORS consumer to achieve the employment goal on an approved Individualized Plan for Employment. Home modifications may be authorized to the extent necessary to facilitate the consumer’s ability to enterand exit their home; access to one bathroom and/or access within the home.

Program Requirements

• All home modification services must be on an approved Individual Plan for Employment (IPE) and have the appropriate level of Administrative Approval.
• Home Modifications – Preliminary Information Form (RS-6u) must be completed and attached to AWARE™.
• Consumer or family member must own the home.
• All home modification services require an AWARE™ Authorization drafted and issued by Central Office, or their designee.
