DORS Internships

The following forms, fact sheets and policy and procedural statements to support DORS staff in developing internships for consumers:

RSM 2, Section 718, Internships describes internship requirements, related fact sheets and forms, Workers’ Compensation coverage, stipends, and documentation of progress in the internship.
DORS Staff Fact Sheet: Unpaid Work-Based Learning Experiences/Internships (RS-7n) outlines procedures in setting up an unpaid internship for a consumer.
Employer Fact Sheet: Internships/Unpaid Work-based Learning Experiences (RS-7p) explains internship requirements to employers who may be interested in hosting internships.
Internship Agreement: Unpaid Work-Based Learning Experiences (RS-7m) is required to clarify details of the internship for the consumer/intern, DORS counselor and internship provider/employer.
Internship Assessment (RS-7r) is provided to the internship supervisor to document progress of the DORS consumer/intern on a monthly basis.
Attendance Log (RS-7s) is for the DORS consumer/intern to document their attendance; requires signature of the internship supervisor and is provided to the DORS counselor on a monthly basis.
