​Required Staff Forms

• Publicity Release Form (RS-9i)
  This form must be filled out and signed by any​​one other than DORS staff who will be photographed or video taped to indicate that DORS has their permission to use the photos/​video. Parent/Legal Guardian must sign for those under the age of 18.

• DORS Staff Presenter/Panelist/Exhibitor Request for Approval​ (on Alchemer)
  When an external organization requests the participation of DORS staff as a presenter or panelist, or if they will be representing DORS as an exhibitor, DORS staff are required to promptly notify their Supervisor/Designated Approver and the DORS Office of Communications & Accessibility Director by completing this form. Form must be submitted no later than one week prior to the event to ensure all approvals are received.

• DDS Staff Presenter/Panelist/Exhibitor Request for Approval​ (on Alchemer)
  When a community organization requests the participation of DDS staff as presenter or panelist, or if they will be representing DDS as an exhibitor, DDS staff are required to promptly notify their Supervisor and the DORS Office of Communications & Accessibility Director by completing this form.
