Request DORS Services

Before You Begin:​

For a high school or college student, aged 14 to 21, please read this Pre-ETS or Student ​Employment Services info ​first.

To request DORS services for yourself or a family member:

(or, if you are 55 years or older and are blind/vision impaired, independent living services)​

• Complete and submit the form below.

• Please allow 10 business days for your nearest DORS Office to receive and process your form and set up an appointment (virtual or in person) for you to meet with a DORS counselor.

• You are welcome to bring a support person, such as a family member, friend or advocate, any DORS appointment or meeting with DORS staff.

• ​​​​Call your nearest DORS Office and ​we’ll help you complete the form over the phone.
  

If you are interested in independent living services but are not over age 55 with a vision impairment:

• ​P​lease contact the Center for Independent Living in your area.
