Request DORS Services

Before You Begin:​

For a high school or college student, aged 14 to 21, please read this Pre-ETS or Student ​Employment Services info ​first.

To request DORS services for yourself or a family member:

(or, if you are 55 years or older and are blind/vision impaired, independent living services)​

  • Complete and submit the form below.

    • Please allow 10 business days for your nearest DORS Office to receive and process your form and set up an appointment (virtual or in person) for you to meet with a DORS counselor.

    • You are welcome to bring a support person, such as a family member, friend or advocate, any DORS appointment or meeting with DORS staff.

The Vocational Rehabilitation program receives 78.7% of its funding through a grant from the U.S. Department of Education. For the Federal fiscal year 2025 the total amount of grant funds awarded was $62,023,386. The remaining 21.3% of the costs ($16,786,507) were funded by State appropriations.
